Minimum Requirements

  • Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the American Physical Therapy Association or equivalent accrediting body.
  • Current, active, and unencumbered Physical Therapist license for the state of assignment prior to start.
  • Basic Life Support (BLS) certification from American Heart Association or American Red Cross may be required.
  • Demonstrated ability to perform patient evaluations, develop and update plans of care, and document clinical findings in the patient health record.
  • Reliable transportation, valid driver’s license, and agency-required liability insurance for performing home visits.
  • Experience supervising or delegating to Physical Therapy Assistants and Home Health Aides preferred.
  • Familiarity with electronic documentation, basic computer navigation, email, and online training systems.
  • State- or client-specific compliance with licensing, supervision, and scope-of-practice regulations as required by the assignment.


Preferred Qualifications

  • 2–3 years of recent physical therapy experience, preferably in home health or skilled home-care environments.
  • Prior travel or agency-based experience supporting continuity of care across multiple patients and settings.
  • Experience conducting home environment evaluations and providing family or caregiver education.


Assignment Details

  • Shift: Evenings, 5 shifts per week, 8 hours per shift.
  • May be required to perform home visits across the service area. Reliable transportation is required.
