Seven Healthcare are seeking a dedicated Physical Therapist for a contract position in Pueblo, CO. This role involves providing comprehensive physical therapy evaluation and treatment based on physician referrals in a supportive environment.

Key Responsibilities of the Physical Therapist
  • Perform physical therapy evaluations and treatments based on physician orders/referrals.
  • Complete a comprehensive evaluation using appropriate assessment tools and procedures.
  • Review patient records for current diagnosis, past medical history, precautions, and contraindications.
  • Establish measurable goals and develop a treatment plan for maximum independence/function.
  • Regularly re-assess treatment plan effectiveness and patient progress.
  • Communicate patient needs and progress to the treatment team, physician, and family members.
  • Consult with medical providers regarding patient progress and treatment discharge.
  • Recommend and assist in securing special adaptive equipment to enhance rehab programs.
  • Maintain and submit documentation in required formats and timelines.
  • Adhere to standards of the Company's Code of Business Conduct and applicable laws.
  • Supervise Physical Therapist Assistants and Aides.
  • Participate in internal staff development and obtain required continuing education.

Requirements of the Physical Therapist
  • PT Doctorate License
  • Fluency with point care systems
