Minimum Requirements

• Degree in Physical Therapy (Bachelors, Masters, or Doctoral) from an accredited program approved by the APTA.
• Active Ohio Physical Therapy license required to start the assignment.
• BLS certification from the American Heart Association or American Red Cross may be required.
• Home Health experience required. Must be able to manage caseload in patient homes and document according to home health standards.
• Ability to meet productivity expectation of 35 points per week.
• Reliable transportation and ability to travel within assigned territory.
• Minimum of 1 year of clinical PT experience preferred.

• Shift Details: Days, 8-hour shifts, Monday through Friday.
