Job Overview

We are seeking a Physical Therapist (PT) for a 14-week contract assignment in Rochester, New Hampshire. The Physical Therapist will evaluate and treat patients recovering from injury, surgery, illness, or mobility impairments within a skilled nursing and rehabilitation setting.

The ideal candidate will have strong clinical assessment skills, experience developing individualized rehabilitation programs, and the ability to collaborate effectively with interdisciplinary healthcare teams to help patients restore function, mobility, and independence.


Key Responsibilities

Evaluate patients and develop individualized treatment plans
Provide therapeutic exercises, mobility training, and rehabilitation interventions
Monitor patient progress and modify treatment plans as needed
Maintain accurate and timely clinical documentation
Educate patients and families regarding treatment goals and home exercise programs
Collaborate with physicians, nursing staff, and rehabilitation teams
Ensure compliance with facility policies, safety standards, and regulatory requirements
Assist patients in improving strength, balance, coordination, and functional mobility


Requirements

Active Physical Therapist (PT) license required
Graduate of an accredited Physical Therapy program required
Current CPR/BLS certification preferred or required by facility standards
Previous skilled nursing or rehabilitation experience preferred
Strong communication, organizational, and patient care skills
Ability to manage patient caseloads effectively in a fast-paced clinical environment
Knowledge of rehabilitation techniques, patient safety standards, and therapy documentation practices

Facility photo badge must be worn at all times during assignment
