Administers comprehensive physical therapy modalities in the home setting. The qualified therapist assists the physician in evaluating the level of function. Helps develop the plan of care and revises it as necessary. Prepares clinical and progress notes. Advises and consults with the family and other personnel. Participates in in-service programs. Relationship-focused interactions include patients, families, staff, and physicians. Promotes effective working relationships and works as part of a team. Utilizes resources, supplies, and time efficiently to increase the value of services. Communicates positively and respectfully with all external sources, patients, families, and staff. Strives for continuous improvement in all aspects of the physical therapist function. Develops and revises plans of treatment and discharge plans. Provides supervision to ensure patient health, safety, and compliance with the plan of care. Consults with and educates patients, families, and team members regarding disease processes. Implements home therapy programs, including gait training, transfers, and activities of daily living. Evaluates patient progress and makes necessary adjustments to the physical therapy program. Teaches para-professional staff in body mechanics and the use of equipment. Coordinates and supervises the quality of patient care services provided. Interacts with patients and healthcare providers to maintain positive relationships. Supports care management philosophy and collaborative customer-focused planning. Participates in patient conferences with members of the care team. Helps orient new rehabilitation personnel. Completes all appropriate clinical records for compliance with regulations. Maintains current knowledge of community-based services. Demonstrates flexibility and willingness to cooperate with customers and other stakeholders. Ensures problem resolution at the point of contact. Makes constructive recommendations for improving quality of patient care. Ensures patients’ needs are met and responds to customer needs effectively. Completes authorization and release of information forms as required. Ensures compliance with established performance competencies. Monitors results, progress, and takes corrective action on quality indicators.
