Minimum Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program.
- Current Physical Therapist license in Florida required to start the assignment.
- Minimum of 1 year clinical experience as a Physical Therapist. Home health experience strongly preferred.
- Experience providing skilled therapy to older adults and medically complex patients preferred.
- Current CPR/BLS certification required.
- Proficiency with electronic documentation systems and mobile technology.
- Valid driver’s license, reliable transportation, and active auto insurance.
- Ability to travel within the assigned coverage area and adapt to different branch operations.
- Ability to work independently in the field with strong clinical judgment.
Assignment Details
- Facility type: Home Health.
- Shift name: Evenings.
- Shift details: Evenings 2000-0400.
- On-call/weekend requirement: Yes.
- Coverage area: The Villages and surrounding communities; travel between patient homes required. Mileage reimbursed between visits from first patient to last patient.
- Weekly hours: 40 hours guaranteed.

The Villages is approximately 20–30 minutes from Ocala National Forest and within a one to two hour drive of Gainesville and Orlando attractions. The region provides numerous golf courses, walking trails, community recreation centers, and frequent local events, supporting an active, social lifestyle with easy access to regional highways and municipal transit options.
Role Responsibilities
- Perform comprehensive physical therapy evaluations and ongoing reassessments in the home setting.
- Develop, implement, and revise individualized plans of care focused on mobility, balance, strength, endurance, pain management, and functional independence.
- Provide skilled interventions including therapeutic exercise, gait and transfer training, balance activities, stair training, and home safety education.
- Educate patients and caregivers on home exercise programs, fall prevention, assistive device use, and energy conservation.
- Document evaluations, visits, progress notes, and discharges accurately and timely in the agency EMR.
- Coordinate care with interdisciplinary team members and communicate changes in patient condition or safety concerns promptly.
- Meet productivity and documentation expectations while managing a mobile caseload.
Preferred Qualifications
- Prior home health travel assignment experience.
- Strong knowledge of Medicare Part A home health regulations and skilled documentation requirements.
- Experience mentoring or supporting staff during short-term assignments.
- Comfort working with high-acuity or high-risk patient populations.
