Minimum Requirements

  • Doctoral Degree in Physical Therapy from an accredited program. (Client requires Doctorate Physical Therapy)
  • Active Oregon Physical Therapist license required to start the assignment.
  • BLS Certification from AHA or ARC may be required.
  • Home Health experience preferred. (Client preference)
  • Experience with HomeCare Homebase EMR preferred. (Client preference)
  • Ability to perform home health PT visits including SOC, PT evaluation, recertification, ROC, and supervisory visits.
  • Willingness to travel within and provide coverage across Josephine County service area.


Assignment Details

  • Facility Type: Home Health
  • Shift Name: Days
  • Shift Details: Monday–Friday, 0800–1600 (client lists Mon–Fri 0800–1630)
  • Weekly Hour Guarantee: 40 hours
  • Productivity Expectations: 30 points (per client guidance)
  • Mileage reimbursed between visits from first patient to last patient.
  • May be required to provide coverage across the county service area; candidate should be able to travel between patient homes.
