Minimum Requirements

  • Doctoral Degree in Physical Therapy (DPT) from an accredited program. This is required by the hiring manager.
  • Active Oregon Physical Therapist license required prior to start of assignment.
  • Current BLS (Basic Life Support) certification from AHA or ARC may be required.
  • Home health experience preferred. Candidates with HomeCare Homebase EMR experience are preferred.
  • Experience conducting PT visits including SOC, PT evaluations, recertifications, ROC, and supervisory visits preferred.
  • Ability to travel within Jackson County for patient visits. Mileage reimbursed between visits from first patient to last patient.


Assignment Details

  • Facility Type: Home Health.
  • Shift Name: Days.
  • Shift Details: Monday–Friday, 0800–1600 (5x8 schedule noted as 0800–1630 in client notes).
  • Weekly Hours: 40 hour guarantee.
  • Productivity Expectation: 30 points (client-defined productivity metric).
