This role involves providing high-quality physical therapy services in a collaborative healthcare environment. The position requires in-depth knowledge of HCHB and prior experience in physical therapy.

Key Responsibilities of the Physical Therapist
  • Perform physical therapy evaluation and treatment of patients based upon physician orders/referrals
  • Complete a comprehensive evaluation using appropriate tools and procedures
  • Review patient records for diagnosis, medical history, precautions, and contraindications
  • Establish measurable goals and develop treatment plans for maximum patient independence
  • Regularly re-assess treatment effectiveness and modify as needed
  • Communicate patient needs and progress to treatment team and medical providers
  • Recommends and assists in securing adaptive equipment to enhance rehabilitation
  • Maintain evaluation, treatment, and billing documentation as per regulations
  • Supervise Physical Therapist Assistants and Aides in patient care activities
  • Participate in staff development and continuing education programs

Requirements of the Physical Therapist
  • Current WV license
  • Prior physical therapy experience
  • Proficiency with HCHB
