Job Title: Physical Therapist Profession: Therapy/Rehabilitation Specialty: PT Home Health Duration: 13 weeks Shift: Day 5x8-Hour Hours per Shift: 08:00 - 17:00 Experience: Minimum 1 year of home health experience required License: State PT License required Certifications: BLS required Must-Have: Home health experience; HCHB preferred Description: Physical therapist needed for home health coverage. Home health experience is required. HCHB experience is preferred. The schedule includes 5x8 shifts, Monday through Friday. The role involves seeing 6 visits per day. Candidates must have experience in adult health care. First-timers are accepted. Weekend requirements do not apply. Candidates must hold a valid state PT license. BLS certification is required. No on-call requirements. Block schedule accommodations are open to consideration. Local candidates are accepted. There is a strict no-tobacco policy with a nicotine drug screen. Candidates will float within nearby facilities as needed. Two supervisor references are required. All modules are completed during orientation and annually. Modules vary by specialty and are recorded through the designated portal. A positive nicotine test will result in cancellation of the application.
