Job Overview
We are seeking a Physical Therapist (PT) for a contract rehabilitation assignment. The PT will evaluate, develop, and implement individualized treatment plans to improve patient mobility, strength, function, and overall quality of life.
Key Responsibilities
Perform comprehensive patient evaluations and assessments
Develop and implement individualized treatment plans
Provide therapeutic interventions to improve mobility and function
Monitor, document, and adjust patient progress as needed
Educate patients and caregivers on rehabilitation goals and home programs
Collaborate with interdisciplinary healthcare teams
Ensure compliance with clinical standards and regulatory requirements
Maintain accurate and timely clinical documentation

Start Date: 06/15/2026
End Date: 09/14/2026
Shift: Day Shift
Shift Time: 08:00 AM 04:30 PM
Schedule: Monday Friday
Contract Length: ~13 Weeks
Guaranteed Hours: 35 per week
Requirements
Active Physical Therapist license (state required)
Graduate of accredited Physical Therapy program
CPR/BLS certification preferred
Experience in rehab, SNF, outpatient, or acute care settings preferred
Strong clinical assessment and treatment planning skills
Excellent communication and documentation abilities
Valid state/federal photo ID required onsite at all times
Compliance with onboarding and health requirements
