We are seeking a compassionate and motivated Physical Therapist (PT) for a 13-week travel assignment in a Home Health setting.

- Shift: Days (8:00 AM – 5:00 PM)
- Guaranteed Hours: 40 hours per week
- Care Setting: Home Health
Job Responsibilities
- Evaluate patients and develop individualized treatment plans to improve mobility, strength, balance, and overall functional independence.
- Provide skilled physical therapy services in patients' homes.
- Monitor patient progress and modify treatment plans as needed to achieve established goals.
- Educate patients and caregivers on home exercise programs, injury prevention, and safe mobility techniques.
- Maintain accurate and timely clinical documentation in the electronic medical record.
