Minimum Requirements

  • Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
  • Active Colorado PT license required to start the assignment.
  • BLS certification from AHA or ARC may be required.
  • Experience performing home health physical therapy evaluations and treatments based on physician orders/referrals.
  • Ability to complete comprehensive evaluations using appropriate assessment tools and to establish measurable goals and treatment plans.
  • Experience documenting evaluations, treatments, and billing in required formats and timelines in compliance with local, state, and federal regulations.
  • Ability to supervise PT assistants and therapy aides in accordance with state practice act.
  • Valid driver’s license and ability to travel within assigned counties. Counties covered: Pueblo, Fremont, Crowley, Custer, El Paso, Otero, Bent, Huerfano, Las Animas.


Preferred Qualifications

  • Fluent with point-of-care documentation systems. (Preferred)
  • Prior home health experience. (Preferred)
  • Experience consulting with medical providers regarding patient evaluation, treatment, progress, and discharge. (Preferred)


Assignment Details

  • Shift Hours: 0800-1700.
  • Work Days: Monday-Friday.
  • Productivity expectation: 30 points per week.
  • Mileage reimbursement provided.
  • Candidate may be required to cover patients across multiple counties listed in minimum requirements.

Impacting Patient Care

Perform physical therapy evaluations and treatments based on physician orders. Complete comprehensive evaluations, develop measurable goals, and implement treatment plans to maximize patient independence and function. Reassess patient progress regularly and modify treatment as needed. Communicate patient needs and progress to the treatment team, physicians, and families. Recommend and assist in securing adaptive equipment. Maintain compliance with company policies, HIPAA, and applicable laws. Participate in staff development and obtain continuing education as required.
