Minimum Requirements

  • Bachelor’s, Master’s, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
  • Active Wisconsin Physical Therapy license required to start the assignment.
  • BLS (Basic Life Support) certification from AHA or ARC may be required.
  • Experience working in skilled nursing or long-term care settings preferred.
  • Experience with common SNF documentation systems and Medicare/Medicaid documentation expectations preferred.
  • Demonstrated ability to provide skilled physical therapy evaluations, plan of care development, and skilled treatment for geriatric patients.
