We are seeking a compassionate and dedicated Travel Physical Therapist (PT) for a 13-week contract assignment in a Skilled Nursing Facility (SNF). The Physical Therapist will evaluate patients, develop individualized treatment plans, and provide evidence-based therapy services to improve mobility, strength, balance, endurance, and overall functional independence.

Responsibilities- Perform comprehensive physical therapy evaluations and develop individualized treatment plans
- Provide therapeutic exercise, gait training, balance training, transfer training, manual therapy, and functional mobility interventions
- Assess patient progress and modify treatment plans to maximize rehabilitation outcomes
- Educate patients, families, and caregivers on home exercise programs, mobility techniques, fall prevention, and safe transfer strategies
- Complete accurate and timely documentation in accordance with facility policies, Medicare guidelines, and regulatory requirements
- Collaborate with physicians, nurses, occupational therapists, speech-language pathologists, and other members of the interdisciplinary healthcare team
- Participate in interdisciplinary care planning and discharge planning
- Maintain a safe, clean, and organized treatment environment
- Ensure compliance with all federal, state, and facility regulations
- Deliver compassionate, patient-centered care focused on restoring function, promoting independence, and improving quality of life
Requirements- Required: Graduate of an accredited Physical Therapy program
- Required: Active Physical Therapist (PT) license in the state of assignment or ability to obtain
- Preferred: Minimum of 1 year of Skilled Nursing Facility (SNF) or post-acute care experience
- Strong communication, documentation, organizational, and interpersonal skills
- Ability to adapt quickly to new clinical environments and work independently
