The facility provides skilled nursing and rehabilitation services in a post-acute and long-term care environment, with a focus on helping patients restore function and safely transition to the next level of care. As a Physical Therapist, you will work in a collaborative, team-oriented setting alongside occupational and speech therapists, nurses, and medical providers to deliver comprehensive, patient-centered care.
Typical responsibilities include completing thorough physical therapy evaluations, developing individualized treatment plans, and providing evidence-based interventions to improve mobility, strength, balance, and functional independence. You will guide patients through gait training, transfer training, therapeutic exercises, and use of assistive devices, while educating patients and families on home programs and safety strategies. You will participate in interdisciplinary care conferences, contribute to discharge planning, and complete detailed documentation that supports quality care and regulatory requirements.
The caseload is primarily adult and geriatric patients with a mix of short-stay rehab and long-term care residents. Common diagnoses include post-orthopedic surgery recovery, neurologic conditions, deconditioning, cardiopulmonary issues, and other medically complex presentations typical in skilled nursing and post-acute care. Patient ratios are designed to support both productivity and quality, allowing you to build strong therapeutic relationships and focus on functional outcomes.
You can expect a structured workday with scheduled evaluations and treatment sessions, collaboration with the broader care team, and opportunities to participate in clinical projects or quality initiatives. Shifts are generally daytime hours, with some flexibility based on facility needs, and may include weekday and occasional weekend coverage to support patient access to therapy services.
