Job Overview
We are seeking a licensed Physical Therapist to provide high-quality rehabilitative care in an outpatient rehab setting. The therapist will evaluate, plan, and implement individualized treatment programs to improve patient mobility, function, and independence.
This role requires strong clinical judgment, documentation accuracy, and the ability to manage a full caseload across multiple patient populations.
Key Responsibilities
- Perform comprehensive physical therapy evaluations and assessments
- Develop and implement individualized treatment plans
- Provide therapeutic interventions to improve mobility, strength, and function
- Document patient progress accurately and in compliance with facility standards
- Collaborate with multidisciplinary healthcare teams
- Educate patients and caregivers on home exercise programs and recovery goals
- Maintain compliance with all clinical and regulatory guidelines
- Adapt care plans based on patient progress and response to therapy
Requirements
- Active Physical Therapist (PT) license in the state of practice
- Degree in Physical Therapy from an accredited program
- CPR/BLS certification (if required by facility standards)
- Strong communication and patient management skills
- Ability to work independently and manage caseload efficiently
- Prior outpatient or rehab experience preferred
