Minimum Requirements
- Master's or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA. Bachelor’s degree acceptable if candidate meets state licensing requirements.
- Current, active Physical Therapy licensure in Pennsylvania required prior to start.
- Current CPR certification required.
- Current driver’s license, vehicle insurance, and reliable vehicle or documented access to public transportation. Mileage between patient visits will be reimbursed.
- Ability to function in any home situation and provide care in accordance with state practice acts and agency policies.
- Experience with home health settings preferred.
- Experience with HCHB EMR preferred.
- Minimum education requirement: Master’s degree. This is a client requirement.

Job Responsibilities (summary)
- Evaluate patients’ functional status and physical therapy needs and assist in developing the plan of care with the physician and care team.
- Provide physical therapy interventions to relieve pain, restore or maintain function, and report patient responses and changes to supervising clinicians.
- Instruct patients, families, and caregivers regarding the plan of care, functional limitations, and progress toward goals.
- Complete evaluations, develop and document PT plan of care, submit progress notes per policy, and report outcomes and projected frequency of care.
Professional and Compliance Notes
- Must practice within scope of state law and agency policies.
- EMR documentation expectations include timely completion of evaluations and progress notes in accordance with agency guidelines.
- Dress code: Royal blue scrubs as requested by the client.
