Minimum Requirements

• Master's or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA. Bachelor’s degree acceptable if candidate meets state licensing requirements.
• Current, active Physical Therapy licensure in Pennsylvania required prior to start.
• Current CPR certification required.
• Current driver’s license, vehicle insurance, and reliable vehicle or documented access to public transportation. Mileage between patient visits will be reimbursed.
• Ability to function in any home situation and provide care in accordance with state practice acts and agency policies.
• Experience with home health settings preferred.
• Experience with HCHB EMR preferred.

Job Responsibilities (summary)

• Evaluate patients’ functional status and physical therapy needs and assist in developing the plan of care with the physician and care team.
• Provide physical therapy interventions to relieve pain, restore or maintain function, and report patient responses and changes to supervising clinicians.
• Instruct patients, families, and caregivers regarding the plan of care, functional limitations, and progress toward goals.
• Complete evaluations, develop and document PT plan of care, submit progress notes per policy, and report outcomes and projected frequency of care.
