This Home Health Physical Therapist position offers the opportunity to practice in Ashland, Alabama, a welcoming small town in Clay County known for its friendly neighbors, relaxed pace of life, and strong sense of community. You will be part of a home health team dedicated to providing personalized, high-quality care to patients in their homes throughout Ashland and surrounding rural communities. The environment is supportive and collaborative, with nursing, therapy, and ancillary staff working closely together to optimize patient outcomes and keep individuals safely in their homes. In this role, you will manage a field-based caseload, traveling to patients’ residences to conduct comprehensive physical therapy evaluations and follow-up visits. A typical day includes assessing strength, balance, gait, transfers, and home safety; developing individualized treatment plans; implementing evidence-based interventions; and regularly updating goals based on patient progress. You will design and progress home exercise programs, perform functional training, educate patients and caregivers on safe mobility and fall prevention, and make recommendations for assistive devices or home modifications as needed. You will collaborate closely with an interdisciplinary team that may include nurses, occupational therapists, speech-language pathologists, social workers, and home health aides. Together, you will coordinate care plans to reduce hospital readmissions, support recovery following surgery or illness, and help patients manage chronic conditions. Regular communication with primary care and specialty providers is expected to ensure continuity of care and alignment of therapy goals with overall medical plans. The schedule typically includes daytime hours with some flexibility to accommodate patient needs and your travel route. You will plan your day to group visits geographically when possible, allowing for efficient travel across Ashland and surrounding areas of Clay County. Patient volumes are structured to support thorough, high-quality care and timely documentation rather than rushing from visit to visit. While patient census can vary, you can generally expect a balanced caseload of evaluations, routine visits, and reassessments. This role is ideal for a Physical Therapist who enjoys autonomy in their practice, appreciates the ability to spend one-on-one time with patients in their own environments, and wants to develop meaningful, long-term connections with the community.
