Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Oklahoma Physical Therapist license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• OASIS and HCHB experience required.
• Minimum 12 months of home health experience within the past 3 years required.
• Experience working within a defined territory and managing travel caseloads preferred.

• Shift Details: Monday - Friday, 8:00 AM - 5:00 PM, 8-hour shifts, 40 hours per week.
