Minimum Requirements

  • Doctoral, Master’s, or Bachelor’s degree in Physical Therapy from an accredited program approved by the APTA.
  • Active Colorado Physical Therapist license required to start the assignment.
  • PT doctorate license is required per client license requirements.
  • BLS certification from AHA or ARC may be required.
  • Experience working in home health or community-based settings preferred.
  • Productivity requirement: ability to meet 30 points per week as outlined by the client (client-specific requirement).
  • Familiarity with point-of-care documentation systems preferred.
  • Ability to perform comprehensive PT evaluations, develop measurable goals, implement treatment plans, and supervise PTAs and aides in accordance with state practice act.
  • Willingness and ability to travel within the county coverage area including Pueblo, Fremont, Crowley, Custer, El Paso, Otero, Bent, Huerfano, and Las Animas counties as part of caseload.
