Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Indiana PT license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Experience in outpatient orthopedics or general outpatient physical therapy preferred.
• Demonstrated ability to perform comprehensive evaluations, write functional treatment plans, and document per payer and clinic guidelines.
• Experience supervising or mentoring students or support staff preferred.
• Maintain professional licensure and complete state-mandated continuing education.

Assignment Details

• Facility type: Outpatient clinic.
• Shift pattern: 5x8 schedule.
• Duration: 13 week assignment.
• Floating: Candidate may be required to float to other clinic locations as needed.
