Minimum Requirements

  • Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
  • Active Indiana Physical Therapy license required to start the assignment.
  • BLS certification from AHA or ARC may be required.
  • Experience working in skilled nursing facility settings preferred.
  • Ability to provide care for patients with post-acute and chronic conditions, including mobility training and restorative programs.
  • Client request: candidate should be available for schedules of approximately 30-35 hours per week.
  • Client request: candidate must agree to a two-week cancellation policy and be flexible to work weekends during assigned weeks as needed. No paid holidays for this assignment.
  • Reliable documentation skills and experience with electronic medical records preferred.
