Job Overview
We are seeking a licensed Physical Therapist (PT) to provide rehabilitative care within a clinical therapy setting. This role focuses on evaluating patient mobility, developing individualized treatment plans, and delivering evidence-based therapeutic interventions to improve functional outcomes and independence.
The Physical Therapist will collaborate with interdisciplinary teams to support recovery, restore movement, and enhance quality of life for patients.
Key Responsibilities
Evaluate patients to determine functional limitations and therapy needs
Develop and implement individualized treatment plans
Provide therapeutic exercises, manual therapy, and mobility training
Monitor and document patient progress and outcomes
Adjust treatment plans based on patient response
Collaborate with physicians, nurses, and rehabilitation teams
Educate patients and families on therapy goals and home exercise programs
Maintain accurate clinical documentation in EMR systems
Requirements
Active Physical Therapist (PT) license in New Hampshire
State-issued identification required
