Description
American Traveler is seeking a licensed Physical Therapist for a 13-week home health travel assignment in the Albuquerque, NM area, with a Monday–Friday schedule and no prior home health experience required.
Details
- Home health setting serving patients within a 45-mile radius of Albuquerque, including areas such as Tijeras, Rio Rancho, Bernalillo, Bosque Farms, Los Lunas, and Belen
- Monday through Friday schedule with no weekend shifts
- 13-week contract
- Caseload includes initial evaluations, progress notes, 30-day reassessments, and 60-day recertifications in accordance with Medicare and state standards
- OASIS assessments, recertifications, transfers, resumptions of care, and discharge documentation required within agency-defined timeframes
- EMR and OASIS documentation tools used; specific system not identified
- Position requires independent work with minimal daily oversight
Requirements
- Graduate of a PT curriculum accredited by CAPTE, APTA, or CAHEA
- Current and active PT license in the state of practice; license required if applicable
- Current CPR certification
- Two years of clinical PT experience preferred; home health experience a plus but not required
- Basic computer proficiency with willingness to learn EMR and OASIS documentation systems
- Reliable personal transportation, valid driver's license, and personal auto insurance required for home visits
- Strong ability to exercise sound clinical judgment and work independently
- DOB and SSN may be required for consideration
Additional Information
- Responsibilities include developing and implementing individualized plans of care aligned with physician orders and patient needs
- Directly supervises PTAs and Home Health Aides in compliance with state practice acts
- Collaborates with physicians and interdisciplinary team members on treatment adjustments and patient progress
- Educates patients and caregivers on home exercise programs, mobility, and adaptive strategies to promote independence and safety
- Applies ICD-10 coding for accurate and compliant documentation
- Periodically presents in-service programs and contributes to staff education initiatives
- Participates in QAPI activities supporting continuous compliance and clinical excellence
- Mileage reimbursement is not included; pay rates are all-inclusive
