Job Title: Break Pharmacist Profession: Pharmacist Specialty: Break Pharmacist Duration: 8 Shift: Day Shift Auto Offer Hours per Shift: 8 Experience: Preferred one year of technical experience, inpatient clinical pharmacy experience License: Pharmacist license in the state of practice required Certifications: All licensures and certifications needed to fulfill the duties required of a Pharmacist at the state level required Must-Have: Current EPIC experience Description: Provide care to the community by verifying and filling orders for medications. Monitor patient medication therapies and provide drug information. Identify and troubleshoot potential medication safety hazards. Document all interactions for medical review. Supervise and direct support personnel. Process, enter, verify, and distribute medication orders in a timely manner. Ensure compliance with department and hospital policies, regulatory guidelines, and accrediting standards. Actively manage medication therapy to ensure efficacy, safety, cost-effectiveness, and optimal therapeutic outcomes. Conduct and manage daily reports on medication utilization to analyze areas for improvement. Consistently perform quality assurance procedures in related work areas. Participate in pharmacy-related tasks and programs such as clinical rounds and medication management activities. Expedite necessary changes to meet regulatory and licensing standards. Comply with drug formulary and ensure appropriate documentation. Collaborate to purchase, contract, and procure medications and related supplies. Assist in making recommendations for process improvement or the development of new pharmacy services. Perform other job-related duties as assigned.
