Job Title: Pharmacist Profession: Pharmacy Specialty: Pharmacist Duration: 26 weeks Shift: Day Shift Hours per Shift: 10 hours Experience: 1 to 2 years preferred License: Pharmacist license in the state of practice required Certifications: BCLS required Must-Have: Document of Pharmacy (PharmD) degree preferred Description: Verify and fill orders for medications. Monitor patient medication therapies. Provide drug information. Identify and troubleshoot potential medication safety hazards. Document all interactions for medical review. Supervise and direct support personnel. Process, enter, verify, and distribute medication orders within the institution in a timely manner. Ensure compliance with department and hospital policies, as well as regulatory guidelines. Actively ensure that medication therapy is efficacious, safe, cost-effective, and achieves the best therapeutic outcomes. Assure that medication prescribing is optimal with respect to indication, dose, route, frequency, and history of allergy. Ensure no duplication or negative interactions with other drugs. Conduct and manage daily reports on medication utilization. Identify, catalogue, and analyze areas needing medication improvement. Perform quality assurance procedures in related work areas. Participate in pharmacy-related tasks and programs. Expedite changes needed to meet regulatory, accreditation, licensing, and agency standards. Participate in department cost savings initiatives. Comply with drug formulary and appropriate documentation. Collaborate with leadership and buyers to procure medications and related supplies. Make recommendations on process improvement or development of new pharmacy services. Perform other job-related duties as assigned.
