Job Title: Pharmacist Profession: Pharmacy Specialty: Pharmacy Duration: 13 weeks Shift: 07:30-18:00 Hours per Shift: 10 Experience: Minimum of 2 years License: Must have state pharmacy license Certifications: BLS certification through the American Heart Association Must-Have: - Recent pharmacy experience in a hospital setting preferred - Knowledge of patient population served Description: The Pharmacist works in collaboration with the Director of Pharmacy according to standard procedures. Compounds and dispenses medications and pharmaceutical supplies upon receiving written prescriptions from the physician. Participates in performance improvement plans within the organization. Consults with and advises administration, medical staff, and other personnel on quality, regulatory, and risk issues. Supports and serves as a consultant to all clinical and medical staff. Assists the Director in coordinating and implementing pharmacy activities. Interprets medication orders and composes and dispenses medications and other pharmaceutical preparations. Supervises technical support staff. Assists with clinical, quality management, and education programs. Provides support for all departments with respect to patient care for adults and geriatric populations. Participates in the maintenance and dissemination of drug information through in-service education. Consults with patients and hospital staff regarding medication therapy. Assists other pharmacists and technicians in routine duties as necessary. Directs activities and provides professional and technical guidance to pharmacy personnel. Exercises professional judgment in imparting therapeutic and drug information to patients and health professionals. Ensures competency in the range of treatments needed for the patient population served. Capable of entering a minimum of 40 orders per four-hour shift. Generates required reports and performs backup procedures accurately. Assists technicians in performing technical tasks and maintains aseptic techniques. Checks technician-prepared products prior to release. Ensures proper maintenance of IV refrigerator temperatures and cleaning logs. Fills orders with correct medications and checks support staff for accuracy before medication release. Prioritizes work to prepare STAT, Now, and Waiting orders based on importance. Reviews patient profiles to ensure appropriate and safe drug therapy. Prevents therapeutic duplication and significant drug interactions. Monitors pharmacokinetics and modifies doses appropriately. Writes progress notes as per protocol and dates and initials all vials for reuse. Ensures accurate labeling of containers, including auxiliary labels. Maintains security of the pharmacy and controlled substances. Informs staff of pending issues and monitors all activities effectively. Keeps the Director informed of workload status and prioritizes work effectively. Follows through on medication orders and fills them upon arrival.
