The CPS will collaborate with medical directors, nurse case managers, physicians, and other providers. Responsibilities include the provision of coordinated, high-quality, evidenced-based, and cost-effective care. The CPS will engage in direct patient care through the Medication Therapy Management (MTM) program. Population-based disease management and specialty pharmaceutical acquisition and care coordination are also key responsibilities. Drug utilization review and clinical policy development and application are included in the role. Participation in Pharmacy and Therapeutics (P&T) Committee meetings is required. The CPS must review pharmacy and medical claims and provide education to members and caregivers. Developing medication-related problem lists and implementing medication action plans are essential tasks. The role includes communicating therapeutic recommendations to physicians effectively. Personal accountability for maintaining professional development and ongoing education is necessary. The environment is fast-paced, complex, and constantly changing. The CPS will enhance the quality of care and optimize outcomes while managing costs effectively. Patient populations served include neonates, pediatrics, adolescents, adults, and geriatrics. The CPS must coordinate and provide comprehensive clinical pharmacy services. Assessment and identification of medication-related problems based on patient evaluation and history are required. Development and execution of patient-specific medication-related action plans are expected. Collaboration with providers regarding results and recommendations is crucial. Tracking ongoing outcomes of clinical interventions and performing routine follow-up to detect new problems is required. Regular communication of clinical pharmacy functions to multidisciplinary team members is important. Educational activities and programs for health plan professional staff are part of the CPS role. Functions will include medical drug policy generation, review, and application. The CPS will serve as a resource to physicians and nurses requesting clinical pharmacy support. Participation in quality assurance evaluations of clinical pharmacy services is necessary. Expertise in Microsoft applications and ability to analyze complex datasets is required. Participation in educational activities including patient education and provider presentations is expected. Actively participating in research findings presentations and professional publications is necessary. Continuing improvement of clinical pharmacy practices through involvement in professional organizations is encouraged.
