Job Summary (List Format): Occupational Therapist (OT)

- Evaluate residents functional abilities, including activities of daily living (ADLs), mobility, cognition, and psychosocial status.
- Develop and implement individualized, evidence-based treatment plans in accordance with evaluation findings and physician orders.
- Deliver direct occupational therapy services in compliance with state and federal regulations as well as facility policies.
- Monitor, assess, and document resident progress, updating treatment plans as needed to achieve rehabilitation goals.
- Collaborate and participate in interdisciplinary care planning meetings to ensure optimal resident outcomes.
- Educate residents, families, and caregivers on techniques to enhance safety and functional independence.
- Recommend and train on adaptive equipment and assistive devices to support resident needs.
- Supervise and provide guidance to Certified Occupational Therapy Assistants (COTAs) and therapy aides as appropriate.
- Maintain accurate, timely, and compliant documentation in the electronic medical record system.
- Stay current with licensure and continuing education requirements as mandated by the state.
- Support quality improvement initiatives and ensure adherence to CMS and facility guidelines.
- Work within a skilled nursing/long-term care environment, often requiring physical tasks such as standing, lifting, and transferring residents in a collaborative, fast-paced setting.
