Key Responsibilities- Screen and evaluate patients with cognitive and/or ADL physical functioning disorders and develop appropriate plans of care in accordance with regulatory and clinical practice standards.
- Provide rehabilitative, skilled, and medically necessary treatment interventions including ADL retraining, therapeutic activities, therapeutic exercises, modalities, and use of supportive and assistive devices.
- Assess and adjust patient treatment plans based on progress, evaluate treatment outcomes for effectiveness, and make referrals as appropriate.
- Provide consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders; communicate effectively with the health care team regarding patient progress and treatment plans.
- Complete all required documentation in the electronic documentation system, accurately record treatment minutes, and provide comprehensive discharge summaries per regulatory and clinical requirements.
- Supervise occupational therapist assistants, rehabilitation aides, and/or students; participate in patient care conferences, utilization review meetings, and required facility trainings.
Required Qualifications- Active Occupational Therapist (OT) license in the state of Oklahoma required at time of submission.
- Minimum of 1 year of Occupational Therapy experience required.
- Long Term Care (LTC) or rehabilitation setting experience required.
- Adherence to NBCOT and state-required practice acts, code of conduct, and ethics standards.
Preferred Experience / Skills- New graduates will be considered provided they have completed a Level II fieldwork experience (Level I observation experience alone does not qualify).
- Experience utilizing standardized assessments and evidence-based practice to support clinical interventions.
