Contract | Days | Full-Time | 5x8-Hour Shifts (8:00 AM – 4:30 PM) | Onsite / Field-Based
Key Responsibilities
Provide skilled occupational therapy assistant services to home health patients in a 1:1 care setting, visiting up to 6 patients per day
Meet a standard productivity expectation of 30 points per day
Complete clinical documentation using EPIC Remote Client and Rover mobile application, including Oasis assessments
Travel to patient homes within the service area, with an occasional maximum radius of approximately 60 miles and up to 90 minutes of total drive time per day
Maintain professional business casual attire while representing the organization in the field
Required Qualifications
Active Oregon COTA license (required prior to start)
Minimum of 1 year of skilled home health experience
Current Basic Cardiac Life Support (BCLS) certification
Primary COVID-19 vaccination series plus one booster (required)
Reliable personal transportation (required for daily patient visits)
Preferred Experience / Skills
Prior experience with Oasis documentation in a home health setting
Familiarity with EPIC Remote Client or similar electronic health record platforms used in home health
Comfort working independently in a field-based, community care environment
