Key Responsibilities
  • Assess patients' level of functioning upon physician orders and plan, implement, and adjust individualized treatment plans based on evaluation results and patient and family goals.
  • Participate in the interdisciplinary team process, including team conferences, collaborative care planning, and discharge planning activities such as placement coordination, patient and family education, and adaptive equipment recommendations.
  • Complete all required documentation including initial evaluations, daily progress notes, treatment plans, discharge disposition, and equipment training records in accordance with department policy and defined timeframes.
  • Supervise Certified Occupational Therapy Assistants (COTAs) and OT/COTA students in the application of occupational therapy theory and techniques; assist with new staff orientation.
  • Promote patient and employee safety across key areas including safe patient handling, materials handling, infectious disease protocols, slip/trip/fall prevention, and overall work environment safety.
  • Maintain flexibility in patient scheduling, communicate effectively with patients and families at their level of understanding, and demonstrate a customer service-oriented approach at all times.
Required Qualifications
  • Current and valid Occupational Therapist licensure in the state of Arizona.
  • Demonstrated ability to perform comprehensive OT evaluations and develop individualized treatment plans.
  • Knowledge of applicable federal, state, and local standards, regulations, and guidelines governing occupational therapy practice.
  • Ability to supervise COTAs and students in a clinical setting.
  • Strong documentation skills with the ability to complete clinical records accurately and within required timeframes.
Preferred Experience / Skills
  • Experience working within an interdisciplinary rehabilitation team environment.
  • Familiarity with discharge planning processes including adaptive equipment recommendation and caregiver training.
  • Demonstrated commitment to continuous quality improvement and patient safety initiatives.
