Occupational Therapist (OT) – Home Health

Schedule / Job TypeContract | Days | Includes weekend rotation (Saturday and Sunday as applicable)
Key Responsibilities- Assess, plan, treat, evaluate, and implement a Plan of Care (POC) for all assigned home health patients
- Practice under AOTA standards of care and code of ethics, collaborating with physicians and the broader healthcare team
- Provide supportive supervision of OTAs and OT/OTA students
- Complete timely and accurate documentation of all patient care activities in the electronic medical record
- Maintain the highest standards of patient privacy, confidentiality, and safety in accordance with established conduct standards
- Apply evidence-based interventions and pursue continuing education to promote quality of care and professional development
Required Qualifications- Minimum of 1 year of Home Health Occupational Therapy experience
- Active Illinois Occupational Therapist license at time of submission
- BLS certification (American Heart Association) – must be active at time of submission
- Valid driver's license and current auto insurance
- COVID-19 vaccination required (declinations will not be approved)
Preferred Experience / Skills- OASIS and start-of-care experience strongly preferred
- Familiarity with home health admission processes, treatment documentation, and appropriate billing practices
