Occupational Therapist – Home Health

Schedule / Job TypeContract | Full-Time | Onsite | Days (Monday–Friday, 8:00 AM–5:00 PM; occasional evenings and weekends as needed)
Key Responsibilities- Evaluate patients and establish individualized plans of care for the home environment; coordinate care with other disciplines and obtain orders for additional services as appropriate.
- Provide direct patient care in accordance with established care plans and physician orders, applying evidence-based occupational therapy practice guidelines, Medicare regulations, and Conditions of Participation.
- Complete and master scoring of CMS-required OASIS assessments, including 30-day functional reassessments, and collaborate with the Quality team on necessary adjustments.
- Perform point-of-care documentation for all skilled services, adverse events, and relevant communications in the medical record; maintain accurate time logs and mileage records.
- Participate in multidisciplinary conferences and communicate with physicians and care team members to support appropriate, patient-centered care planning, including admissions, OASIS time points, and discharges.
- Supervise Occupational Therapy Assistants and CNAs as required, completing supervisory visits within required timeframes and participating in Quality Assurance Performance Improvement initiatives.
Required Qualifications- Bachelor of Science degree or higher from an accredited Occupational Therapy program.
- Current Occupational Therapist license to practice in the State of Montana.
- Valid driver's license and appropriate personal auto insurance.
- Working knowledge of Medicare regulations, Conditions of Participation, and OASIS documentation requirements.
- Ability to meet productivity standards and manage an efficient home visit schedule.
Preferred Experience / Skills- Minimum of two years of occupational therapy experience, preferably in a home health setting.
- Familiarity with home health insurance requirements and payer guidelines.
- Experience supervising Occupational Therapy Assistants or CNAs.
