Key Responsibilities- Evaluate patients and develop individualized occupational therapy treatment plans within a skilled nursing facility setting
- Provide direct occupational therapy services to patients to restore, maintain, or improve functional abilities
- Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory standards
- Collaborate with interdisciplinary care teams to coordinate and optimize patient care
- Educate patients and their families on therapeutic techniques and strategies to support recovery and independence
Required Qualifications- Graduate of an accredited Occupational Therapy program
- Current and valid Occupational Therapist licensure in the state of Hawaii (or eligibility to obtain)
- Current NBCOT certification
- Experience working in a skilled nursing facility (SNF) setting
- Strong documentation and communication skills
Preferred Experience / Skills- Previous travel or contract therapy experience
- Familiarity with electronic medical records (EMR) systems used in SNF environments
