Key Responsibilities- Contribute professional and clinical knowledge as a member of an interdisciplinary therapy team to provide and manage patient care in a home health setting
- Provide occupational therapy services to patients primarily requiring short-term therapy following hospitalization or nursing home stays
- Travel within approximately 50 miles of the Fort Dodge area to conduct home visits, averaging 200–250 miles per week
- Document patient care using an electronic medical records system (EPIC)
- Participate in weekly virtual team meetings and monthly in-person team meetings
- Collaborate with on-staff therapy personnel including PT, PTA, and COTA team members
Required Qualifications- Minimum of 2 years of occupational therapy experience
- Active Iowa state OT license
- Current BLS certification
- Valid driver's license and personal vehicle (mileage reimbursed)
- Must maintain a taxable address located more than 50 miles from the assignment location
- Must not have been employed by the facility within the last 12 months
Preferred Experience / Skills- Prior home health experience (department is willing to train candidates without it)
- Experience with EPIC EMR
