(NBCOT) - Valid licensed driver with automobile insurance in accordance with state and organizational requirements - Mandatory Reporter: Child/Dependent Adult Abuse Description: As an Occupational Therapist in our Home Health team, you will bring your clinical expertise and compassionate care directly to patients in the comfort of their own homes. You will play a key role in developing and delivering personalized therapy plans that help patients regain independence, improve their quality of life, and achieve their goals. Working closely with physicians and the care team, you will provide hands-on treatment, education, and support, all grounded in best clinical practices. Responsibilities include making visits to the homes of patients requiring home health occupational therapy services and assuming responsibility for an ongoing interdisciplinary assessment of the patient. You will evaluate and assess the patient's level of functioning by applying appropriate tests. You will develop specific occupational therapy procedures as well as functional activities in accordance with a written physician plan of treatment and patient evaluation. Additionally, you will develop and administer a plan of treatment including environmental modifications and oversee the use of treatment modalities, prosthetic devices, and therapeutic appliances.
