Key Responsibilities- Evaluate and treat patients in a skilled nursing facility setting to restore or improve functional independence
- Develop and implement individualized occupational therapy treatment plans based on patient needs and goals
- Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory standards
- Collaborate with interdisciplinary care teams to coordinate patient care and discharge planning
- Educate patients and caregivers on therapeutic techniques and adaptive strategies to support daily living activities
Required Qualifications- Graduate of an accredited Occupational Therapy program
- Current and valid Occupational Therapist (OT) licensure in the state of Ohio
- Current NBCOT certification
- Prior experience working in a skilled nursing facility (SNF) setting preferred
- Current CPR/BLS certification
Preferred Experience / Skills- Experience with geriatric patient populations in a post-acute or long-term care environment
- Familiarity with Medicare documentation and compliance requirements in a SNF setting
