Pride Health is hiring a Medical Assistant to support our client's patients in Centralia, WA (Coverage: 50-mile radius). This is an 11-week assignment (Possibility of extension or conversion) and a great way to start working with a top-tier healthcare organization!
 
Job Details:
Job Name: Home Health Occupational Therapist
Contract type: Local Contract
Location: Centralia, WA (50-mile radius)
Duration: 11+ week Contract (Possibilities of extension)
Pay rate: $48-53/hr. On W2 (Pay Rate is based on experience)
Schedule: Mon – Fri (5 days/wk.) 8 am to 4:30 pm (40 hrs. guaranteed)
Mileage: Reimbursement provided (from first patient to last patient)
 
Job Responsibilities:
  • Conduct comprehensive occupational therapy evaluations and patient care assessments in the home health setting.
  • Develop individualized treatment plans based on patient needs, physician orders, and functional goals.
  • Complete recertifications and reassessments to evaluate patient progress and determine continued eligibility for services.
  • Perform discharge evaluations and provide recommendations for ongoing care, home safety, adaptive equipment, and caregiver education.
  • Deliver therapeutic interventions focused on improving mobility, strength, ADLs, safety awareness, and independence within the home environment.
  • Maintain timely and accurate clinical documentation in accordance with agency, state, and federal guidelines.
  • Collaborate with physicians, nurses, case managers, and interdisciplinary care teams to coordinate patient care.
  • Educate patients and caregivers on therapy exercises, home modifications, fall prevention, and use of adaptive equipment.
  • Ensure compliance with Medicare home health regulations and agency standards.

    Qualifications:
    • Current Occupational Therapy license in the state of practice.
    • Home Health experience required.
    • Experience with Homecare Homebase required.
    • Current CPR certification.
    • Valid driver’s license, auto insurance, and reliable transportation.
