Job Title: Occupational Therapist Profession: Occupational Therapist Specialty: - Duration: 13 weeks Shift: M-F Hours per Shift: 8:00 AM - 4:30 PM Experience: Minimum of two years in a clinical setting with a frail or elderly population License: Licensed or eligible for licensure as required in the state Certifications: Initial registration from the National Board for Certification in Occupational Therapy; BLS Must-Have: - Excellent leadership, organizational, and communication skills in settings with seniors, their families, and interdisciplinary team members - Ability to work independently with minimal supervision - Experience leading in a data-driven organization Description: The Occupational Therapist is responsible for coordinating and providing restorative and rehabilitative therapy services to participants in the center and in their homes. Occupational Therapy works closely with the physician, rehabilitation staff, and other interdisciplinary team members to maximize participant independence and safety. The Occupational Therapist collaborates with the Physical Therapist to efficiently operate the rehabilitation department. Conducts assessments to determine the therapy care needs, preferences, and goals of participants. Actively participates in interdisciplinary team meetings to develop participant care plans. Delivers and documents therapy interventions in accordance with physician orders and agreed-upon care plans. Utilizes devices, techniques, and activities to promote muscle re-education, perceptual-motor and cognitive function, balance, endurance, and factors affecting independence in performing activities of daily living (ADLs). Supervises therapy assistants and provides training, instruction, and guidance needed to implement Functional Maintenance Programs. Trains and instructs staff, participants, and caregivers in occupational therapy practices. Orders, tracks, and manages maintenance for equipment. Remains alert to input from team members, participants, and caregivers and promptly updates the interdisciplinary team of any changes in participants' conditions or functional status. Supports smooth care transitions between settings such as hospitals, skilled nursing facilities, and home. Participates in end-of-life care coordination and support. Manages direct reports, including training, supervising, and mentoring. Completes all required documentation in a timely and accurate manner. Protects privacy and maintains confidentiality of all procedures and information regarding team members, participants, and families. Follows policies and procedures and participates in required quality improvement activities, staff training, and meetings. Communicates regularly with the supervisor and team regarding workload and priorities. Exercises flexibility in performing assignments as business needs evolve. Performs other duties as assigned.
