Location: Hood River, Oregon and surrounding areas
Schedule: Day shift, 8-hour shifts (08:00 - 16:30), Monday through Friday. No weekends required. May visit up to 6 patients per day with a standard productivity expectation of 30 points. This position requires 1:1 patient care in home health settings.

Requirements:- Active Oregon and Washington occupational therapy licenses required (Washington license in hand preferred; Oregon may be pending)
- Minimum 1 year of skilled home health experience required
- OASIS documentation experience required
- Current BLS/BCLS certification
- Reliable personal transportation required for patient visits
- Experience with EPIC charting system preferred
- Proficiency in functional activities, acute and chronic disorder management, adaptive equipment, and patient/family education
