Key Responsibilities
- Conduct occupational therapy evaluations and develop individualized treatment plans
- Provide skilled OT interventions including ADL/IADL training, fine motor, cognitive rehabilitation, and adaptive equipment
- Document patient progress and maintain accurate records in the EMR
- Supervise COTA and rehab aide staff as applicable
- Collaborate with interdisciplinary team on discharge planning and home recommendations
- Meet productivity standards and participate in quality improvement activities
Requirements
- Active state OT license (or eligibility for endorsement)
- CPR/BLS certification current
- Minimum 1 2 years of OT experience in a SNF/LTC or post-acute setting
- Proficiency with EMR documentation and productivity tracking
- Strong interpersonal and interdisciplinary collaboration skills
- Active New Mexico Caregivers Registry listing required for placement at NM facilities
