Job Overview

We are seeking a skilled Occupational Therapist (OT) for a travel assignment across two rehabilitation settings. The ideal candidate will evaluate, plan, and implement occupational therapy interventions that help patients regain independence in daily living activities.

This is a shared role split between Linden Grove Health and Washington State Soldiers Home, requiring flexibility and strong clinical adaptability.


Key Responsibilities

Evaluate patients functional abilities and occupational performance
Develop and implement individualized treatment plans
Provide therapeutic interventions to improve ADLs (activities of daily living)
Monitor and document patient progress and adjust care plans as needed
Educate patients and caregivers on adaptive techniques and equipment use
Collaborate with interdisciplinary teams including OT, PT, nursing, and physicians
Maintain accurate and timely clinical documentation
Ensure compliance with facility and regulatory standards

Start Date: 06/01/2026
End Date: 08/29/2026

Shift: Day Shift
Shift Time: 08:00 16:30

Schedule: Monday Friday (Shared Facility Rotation)
Contract Length: Approximately 13 Weeks


Requirements

Active Occupational Therapist (OT) license required
Degree in Occupational Therapy (Bachelor s/Master s depending on jurisdiction)
Prior rehab or skilled nursing experience preferred
Strong clinical judgment and treatment planning skills
Ability to work across multiple facilities
Must comply with onboarding and facility compliance requirements
Must bring state or federally issued identification and wear a photo badge at all times
