Job Title: Occupational Therapist Profession: Occupational Therapist Specialty: Long Term Care Duration: 13 weeks Shift: Days Hours per Shift: 8 hours Experience: 1+ years of experience License: Active Occupational Therapy license Certifications: N/A Must-Have: LTC or Rehab Experience Description: Effectively screens and evaluates patients with cognitive and/or ADL physical functioning disorders. Develops an appropriate plan of care following all regulatory and clinical practice standards. Utilizes standardized assessments and evidence-based practice to support clinical interventions. Provides rehabilitative, skilled, and medically necessary treatment interventions to patients. Administers various procedures as part of the rehabilitation plan, including ADL retraining, therapeutic activities, and therapeutic exercises. Utilizes modalities and supportive or assistive devices as required. Assesses patient progress and adjusts treatments accordingly in the clinical record. Evaluates treatment outcomes for effectiveness and modifies services as needed. Provides consultation and counseling to patients, families, caregivers, and service providers. Effectively communicates with supervisors and health team members regarding patient progress and treatment plans. Documents all regulatory and clinically required information in the electronic documentation system. Maintains patient records according to policy and accurately records treatment minutes. Participates in required meetings, including patient care conferences and utilization review meetings. Engages in training, in-services, and conferences as required. Obtains necessary durable medical equipment or supplies per regulations and clinical practice requirements. Supervises and supports occupational therapist assistants and rehabilitation aides. Adheres to practice acts, codes of conduct, and ethics during professional practice as an Occupational Therapist.
