Job Title: Occupational Therapist Profession: Occupational Therapist Specialty: Home Health Duration: 13 weeks Shift: 8:00 AM - 4:30 PM Hours per Shift: 8 Experience: Minimum of 24 months of experience with recent experience in the last 12 months License: Active license to practice Certifications: BLS certification from approved vendor Must-Have: Home care experience is required. Candidates must be willing to outline all criteria they are willing to accept. Flexibility with locations, shifts, and departments is encouraged. Candidates must be able to start on the listed start date or no more than one week later. Requested time off (RTO) must be included and cannot exceed 7 days. No RTO during the first week of assignment. Candidates should be pre-closed and ready to accept. Immediately inform if the candidate withdraws or is no longer available. Candidates must have no hits on their state license to practice. Candidates must provide a driver's license for residency verification. Travel experience is highly preferred. Description: Home care experience is essential for this role. Candidates must demonstrate flexibility regarding shifts, locations, and departments. All candidates will undergo a thorough background check. Documentation required includes a completed cover page and a resume that lists facilities, departments, trauma level designations, and bed counts. An OT skills checklist must be completed within the last 12 months. Verification of an active license to practice is required within the last 30 days. An active BLS certification from an approved vendor is necessary. A valid driver’s license must be provided.
