Job Title: Occupational Therapist Profession: Occupational Therapy Specialty: Long Term Care Duration: 13 weeks Shift: Days Hours per Shift: 8 hours M-F, flexible hours as long as consistent and communicated with the Director of Rehabilitation Experience: 1+ years of experience preferred, new graduates with Level II experience considered License: Active Occupational Therapy license required Certifications: None specified Must-Have: - 1+ years of relevant experience - Active Occupational Therapy license Description: Effectively screens and evaluates patients with cognitive and/or ADL physical functioning disorders. Develops appropriate plans of care following all regulatory and clinical practice standards. Utilizes standardized assessments and evidence-based practices to support clinical interventions. Provides rehabilitative, skilled, and medically necessary treatment interventions. Administers various procedures as part of the rehabilitation plan, including ADL retraining and therapeutic activities. Assesses patient progress and adjusts treatments accordingly in clinical records. Evaluates treatment outcomes for effectiveness and modifies services based on patient evaluations. Provides consultation and counseling to patients, families, caregivers, and other service providers. Effectively communicates with supervisors and other health team members regarding patient progress and treatment plans. Provides comprehensive discharge summaries of services provided. Documents all required information in the electronic documentation system and maintains patient records according to policy. Accurately records treatment minutes for services provided. Participates in required meetings, including patient care conferences and rehabilitation meetings. Engages in facility and company required training, in-services, and conferences. Obtains necessary durable medical equipment or supplies as per regulatory and clinical practice requirements. Supervises and supports occupational therapy assistants and rehabilitation aides. Adheres to practice acts, codes of conduct, and ethics during professional practice.
