Job Title: Occupational Therapist Profession: Therapy Specialty: Home Health Care Operations Duration: 13 weeks Shift: Day Shift Hours per Shift: 8 hours Experience: Minimum 3 years License: Wisconsin License Certifications: BLS Must-Have: - Bachelor’s degree in occupational therapy - Active driver's license and auto insurance Description: Home health position covering multiple counties. Expectation of 6 points per day. Will work primarily with adult patients. Common diagnoses include geriatrics, post-stroke, post-joint replacement, weakness, deconditioning, COPD, and CHF. Interdisciplinary team collaboration with RN and PT staff. Morning team huddle conducted via phone every morning. Evaluation visits are 1.5 hours; standard visits are 1 hour. General OT treatment and evaluation skills focusing on activities of daily living and adaptive equipment are required. Technology used includes an EMR system. Travel is required within the designated coverage area. Most patients are within a 20-30 minute radius, with occasional visits up to one hour away. No weekend rotation or on-call duties are required. Holiday work is not expected. Approval of requests for time off will go through the manager. Shift times are approximately 7:45 AM to 4:00 PM, Monday through Friday.
