This 13-week Occupational Therapist assignment is based in a skilled nursing facility in Merced, California, a vibrant Central Valley community known for its welcoming feel, growing university presence, and easy access to some of California’s most iconic outdoor destinations. Merced offers a comfortable, relaxed lifestyle with less congestion than major metro areas, while still providing convenient shopping, dining, and cultural events. Outdoor enthusiasts appreciate being within driving distance of Yosemite National Park, scenic lakes, nature refuges, and nearby trails, making it easy to balance meaningful work with weekend adventures. The facility is a dedicated post-acute and long-term care center focused on helping residents regain function, maximize independence, and safely transition along the care continuum. As the Occupational Therapist on this 8-hour day shift schedule, you will provide comprehensive assessment, treatment, and discharge planning for an adult and geriatric population with diverse diagnoses, including post-surgical conditions, orthopedic injuries, neurological events, and chronic medical complexities. A typical day includes performing evaluations, developing individualized treatment plans, leading functional and ADL-focused interventions, and documenting timely, accurate notes in the electronic medical record. You will work closely with physical therapy, speech therapy, nursing, case management, and social services in an interdisciplinary team model that emphasizes collaborative care and clear communication. Patient ratios are structured to support quality, goal-driven therapy and continuity of care throughout the stay.
