Job Title: Occupational Therapist Profession: Occupational Therapy Specialty: Long Term Care Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: 1+ years of experience in occupational therapy; long-term care or rehabilitation experience required License: Active occupational therapist license Must-Have: - Skills checklist within 1 year - 0-1 reference at time of submission Description: Effectively screens and evaluates patients with cognitive and/or ADL physical functioning disorders. Develops an appropriate plan of care following all regulatory and clinical practice standards. Utilizes standardized assessments and evidence-based practice to support clinical interventions. Provides rehabilitative, skilled, and medically necessary treatment interventions to patients with cognitive and/or ADL physical functioning disorders. Administers various procedures as part of the rehabilitation plan, including ADL retraining, therapeutic activities, therapeutic exercises, and modalities. Uses supportive and assistive devices as per patient needs. Assesses patient progress and adjusts treatments accordingly in the clinical record. Evaluates treatment outcomes for effectiveness and modifies services based on patient evaluations. Refers patients as needed based on outcomes. Provides consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders. Effectively communicates with supervisors and other health team members regarding patient progress, barriers, and treatment plans. Provides comprehensive discharge summaries of services in compliance with regulatory and clinical requirements. Documents all required information in the electronic documentation system. Maintains patient records according to company policy. Accurately records treatment minutes for all services provided. Participates in required meetings such as patient care conferences, utilization review meetings, and rehabilitation meetings. Engages in facility and company required training, in-services, and conferences. Obtains necessary DME or supplies per regulatory and clinical requirements. Supervises and supports occupational therapist assistants, rehabilitation aides, and students. Adheres to professional practice standards, codes of conduct, and ethics during practice.
